Part 1 – 11:00 – 13:00 – Proactive Care
The Proactive Care workstream works with the other workstreams within the Primary Care Transformation portfolio – workforce implications, access challenges, PCN development and digital opportunities – to support systems in keeping local populations well for longer and preventing health issues through the implementation of various activities and interventions that bolster the resilience and capacity of the people who support their care across local communities throughout their lifetime.
This session provides an opportunity to share progress in the Locally Commissioned Projects across the region and wider projects tackling health inequalities. This is a fantastic opportunity to learn from regional colleagues.

This session provides an opportunity to catch up on a large number of PC/PCN webinars that have been held since Sept 22. There are lots of topics that have been covered and we will be sharing pre-recorded highlights from them. There will be take away tips to take back to practices and PCNs and a live drop-in session with the speakers on the day so you can bring questions.
