Part 1 – 11:00 – 13:00 – Proactive Care

The Proactive Care workstream works with the other workstreams within the Primary Care Transformation portfolio – workforce implications, access challenges, PCN development and digital opportunities –  to support systems in keeping local populations well for longer and preventing health issues through the implementation of various activities and interventions that bolster the resilience and capacity of the people who support their care across local communities throughout their lifetime.

This session provides an opportunity to share progress in the Locally Commissioned Projects across the region and wider projects tackling health inequalities. This is a fantastic opportunity to learn from regional colleagues.

Topic	Content
Introduction and Scene Setting	An overview of the Proactive Care workstream within the context of Primary Care Transformation Portfolio. A demonstration of the Proactive Care Workstream outputs and successes from 22/23, and a view of Proactive Care Workstream priority objectives and outcomes for 23/24
Primary Care Transformation, Locally Commissioned Proactive Care Projects	An overview from the locally commissioned project tackling Digital Inclusion, Health Literacy, Clinical Coding of patient cohorts and Coastal Deprivation. The project leads with updates with progress of their local projects including next steps. This will include their plans to share the learning and embed these effective practices across the ICB, and we will work to do the same across the regional footprint.
Innovation for Healthcare Inequalities Programme (InHIP) Projects	The AHSN Network are providing support to ICSs to enable the identification and adoption of innovations and technologies that address the five clinical priorities within the national Core20PLUS5 strategy. Overview of Innovation and Good Practice across the region from the InHIP Projects
Regional Innovation and ‘What Good Looks Like’ in Primary Care	Examples of good practice from organic primary care led innovation tackling health inequalities and multi-morbidity’s from across e region.
Q&A & Close	An opportunity to ask further questions about the workstream or any of the topics covered in the webinar

Part 2 – 14:00 – 16:00 – Primary Care and PCN Development

This session provides an opportunity to catch up on a large number of PC/PCN webinars that have been held since Sept 22.  There are lots of topics that have been covered and we will be sharing pre-recorded highlights from them.  There will be take away tips to take back to practices and PCNs and a live drop-in session with the speakers on the day so you can bring questions.
