Leadership Development opportunities at a system level

Collaborative Practice Action Research programme

Collaborative Practice is a new social model of health care, one designed to support people to adapt and cope with long term conditions, loneliness, isolation, anxiety, and old age. It is a change model based in primary care, which helps health services and local people find new ways of working together – making a difference to people’s lives, releasing resources, creates capacity in the system, and improving services at place
