Collaborative Practice programme for GP practices

Collaborative Practice is a data driven approach that supports general practice to understand the nature of demand and to find new ways of organising that will not only reduce the number of repeat appointments but will also meet the needs of patients. Altogether Better help GPs, Practice Managers and their teams to develop the skills, mindset and culture needed to create general practice fit for the future. We support practices to invite people on their list and from their community to work alongside them as volunteer Practice Health Champions, becoming part of a new extended team that tackles the problems that medicine alone cannot fix. Complementing and amplifying the role of the Social Prescribing Link Workers and Care Coordinators, a plethora of new offers and activities emerge that meet social and wellbeing needs.
